Jockey Club Discharge Support and Rehabilitation Programme

A thank you letter from a granddaughter of one of clients that we want to share here. After discharging from the hospital and joining the Programme since two months ago, 90 year-old Ms Sin is now able to walk by herself with the walking aid. Her granddaughter becomes more confident in her ability to take care her grandma at home. As well, with the continuous support of our professional team, she feels self-assured to manage and help alleviate dementia-related behavioural symptoms that her grandma may be prone to exhibit. We are glad that Ms Sin can return home living at her community. Her recovery and her granddaughter’s encouraging words motivate us to continuously look at ways to support people with dementia and their family.

Jockey Club Post-diagnostic Support in Dementia Care Programme

Jockey Club Post-diagnostic Support in Dementia Care Programme (PDSP) has prepared some videos of laughter yoga suitable for elderly to practise even at home with the family. Practising the exercises can increase blood circulation, increase oxygen in the brain and slow down cognitive impairment. Let’s relax, do exercise and laugh loudly together! PDSP aims to provide early screening, early diagnosis and early intervention in dementia. If you observed that the elderly in your family had displayed a significant decline in memory and cognitive impairment, but not sure whether such changes were due to normal ageing or dementia. Please feel free to contact our programme staff to arrange a detailed cognitive assessment. PDSP provides supportive service to people who have diagnosed with Mild Cognitive Impairment (MCI) and early dementia within one year, as well as those family caregivers too. In view of the latest development of the novel coronavirus infection, elderly and the caregivers can choose to receive supportive service via telephone or Zoom video conference.

PDSP provides free educational talk and the cognitive test for different NGOs, organizations and for those who aged 50 or above.

Jockey Club Caring Communities for Dementia Campaign

With the generous support from the Hong Kong Jockey Club Charities Trust, Jockey Club Centre for Positive Ageing (JCCPA) is partnering with St James’ Settlement and The Hong Kong University of Science and Technology to organize the “Jockey Club Caring Communities for Dementia Campaign (CCD)” from September 2019 to August 2022.  With the integration of community network of NGOS and technological expertise, the Campaign aims to provide feasible service for caregivers of person with dementia and promote a dementia friendly community in Hong Kong. With the active participation of the public, people with dementia will be able to continue participating in community activities safely while caregivers’ stress could be relieved.

In order to enhance the awareness of supporting people with dementia in our community, we are conducting a series of public education talks. At the same time, the project team are working together to develop a tracking device and APP to help identifying missing person with dementia. The tracking device and APP is targeted to be launched in the third quarter of 2020.
