Jockey Club Post Discharge Programme for People with Dementia is a project designed to assist hospitalized people with dementia, helping them recover and adapt to home life as soon as possible after discharge. This programme consists of three plans to meet the varying needs of families with dementia.
Background
Jockey Club Centre for Positive Ageing has received funding from The Hong Kong Jockey Club Charities Trust to implement the third phase of the Post Discharge Programme for People with Dementia again. The concept of the programme is based on the “Confinement Rehabilitation Programme” proposed by JCCPA Advisory Board member Ms Angelina Luk, a solicitor. When individuals with dementia are preparing to leave the hospital, their overall physical condition is often very unstable and weak. Family members often feel extremely lost and uncertain when arranging for their loved ones with dementia to return home for recovery. The Post Discharge Programme for People with Dementia aims to encourage and assist individuals with dementia in recovering as quickly as possible after hospitalization so they can return home and age in place. Through a professional team, community collaboration, and resource integration—along with accompanying families and providing relevant training and advice—the program helps patients transition safely from hospital to home life. At the same time, it reduces the burden on the healthcare system, achieving a sustainable, “community-based, person-centered” care model.
Objective
The programme is devised to help people rehabilitate and adapt to living and ageing at home after hospitalization.

Plan 1 | 12 weeks of residential and daycare, including 28 consecutive days of residential service (in a quadruple room) and eight weeks of daycare service (from three days per week), with cognition activities, personal care, rehabilitation training, home evaluation and other services to help participants recuperate | Participants using services within this plan can receive 25%, 50%, or 75% funding, depending on their financial circumstances. Each participant in the daycare and training services can enjoy a transport subsidy of $100 per day. If an upgrade to another room or additional service days are required, the difference must be paid at full price. |
Plan 2 | Six weeks of daycare and training (from two days per week) to help participants improve physical performance and enhance physical capabilities | |
Plan 3 | Professional staff will connect with family members either online or in-person to assess the individual needs of discharged people with dementia, grasping required care techniques and providing psychological education support services in an empowering manner at once. | Free of charge |
Notes:
- All other services not specified in the programme will be charged at full price.
- Participants must pay the fees for included services in advance to commence the services.

Target Audience (must meet all the criteria below)
- People with dementia
- Hospitalized for an acute condition or emergency, and evaluated by a doctor as suitable for discharge
- Able to move directly into JCCPA after discharge
- Will be able to continue living in the community, under the care of family members or a domestic helper, after the residential services at JCCPA
- Able to adapt to living in a group
- Evaluated by JCCPA as physically and mentally fit for residential services
