The concept of the programme is based on the \u201cConfinement Rehabilitation Programme\u201d proposed by JCCPA Advisory Board member Ms Angelina Luk, a solicitor. When individuals with dementia are preparing to leave the hospital, their overall physical condition is often very unstable and weak. Family members often feel extremely lost and uncertain when arranging for their loved ones with dementia to return home for recovery. The Post Discharge Programme for People with Dementia aims to encourage and assist individuals with dementia in recovering as quickly as possible after hospitalization so they can return home and age in place. Through a professional team, community collaboration, and resource integration\u2014along with accompanying families and providing relevant training and advice\u2014the program helps patients transition safely from hospital to home life.
